Partners Hope Foundation was founded in 2017 by a group of concerned community members to address Western Montana’s hospice care needs. Collaborating with Partners In Home Care, we are creating a center for end-of-life care that will be the only facility providing dedicated 24-hour hospice care to patients and families living in Granite, Lake, Mineral, Missoula, Ravalli and Sanders counties. The center will also offer end-of-life education and support services, partnering with regional providers including All Nations Health Center, Tamarack Grief Resource Center, veteran’s organizations and others.
The center will be located on Union Pacific Street near Home Depot off North Reserve in Missoula. The design was informed by extensive input from local healthcare professionals, families who have experienced hospice care and community members. We are proud to present the renderings of this state-of-the-art facility.

Most people would prefer to die at home, unfortunately, for many that is not an option.
Today, Western Montana families have limited options for continuous 24-hour hospice care. Many seniors live alone, live with an aging spouse, or have children and relatives who live far away. Individuals in our marginalized populations often suffer through the declining phases of life.
With your support, the center will provide dedicated hospice care when dying at home is not an option.

Partners Hope Foundation is a qualified 501(c)(3) non-profit organization, and gifts are deductible within IRS regulations.
The center will care for an expected 3,000 PATIENTS in the first five years.
The center will serve patients from the SIX-COUNTY region of Sanders, Lake, Mineral, Missoula, Granite and Ravalli counties.
The center will provide 24-HOUR whole-person hospice care — inpatient, residential and respite.
